Appointments cancelled within 24 hours or no-shows are subject to a cancellation fee of $50 which is not covered by insurance. If there is an emergency, please notify me.
All home visits will be charged a $40 trip fee, to be paid at the time of the consultation, which covers up to 60 miles round trip from Montgomery, Texas. Additional mileage will be charged at 65 cents per mile. This is typically not recoverable by insurance and may not be reimbursable for cash paying patients submitting an insurance claim.
